Google does not value a normal website the same way it values a healthcare website. Google has separate rules for link building and other ranking factors for healthcare websites, which fall under what is called "Your Money or Your Life" (YMYL). Most of the agencies that get paid $500 a month or something basically go ahead and use a bot. That bot will take the link they are given and post it across hundreds of different blog websites that are open for publishing comments. The comments are randomly posted with generic text like:

"Hey, I found this article very interesting. Click the link below to read more."

"I found this article is life-changing and it is giving me anti-aging up to five years."

For a cardiology backlink, the bot might post something like "My heart age is reduced by five years."

For a pain doctor backlink, these bots will put a generic comment like: "Yeah, I love your article. It's really great and detailed. I love the way you presented your article. I also read another article about pain management and how to reduce your chronic pain instantly by following these steps at home." And then a backlink to the pain doctor's website. They create hundreds of comments like that, all backlinking to the pain doctor's website. Then the doctor is told, "Hey, we gained 200+ backlinks."

There are a lot of Web 2.0 directories that accept links, and those links get submitted. They are not local directories. They are all spam link building directories. All 200 links are posted randomly. They are not relevant. They are not informational. They are not backlinked by higher domain authority platforms. They simply post anywhere on the internet that accepts a backlink.

It should not be like that.

The Right Approach to Healthcare Link Building

Instead of building backlinks from random sites, you need to be getting backlinks from healthcare publications and health-related blogs not from technology blogs that talk about AI and other things. Instead of getting a backlink from a domain authority 80 tech blog, you can get a backlink from a domain authority 20 healthcare blog or website. Both are entirely different in terms of value for a healthcare practice.

For example, let's say a medspa wants to build a backlink for their Botox page. Instead of posting backlinks across a lot of websites that accept comments, you can focus on outreaching to five different blogs that talk about Botox in detail educational blogs. You can email them, asking if they can allow you to write content to post on their website.

You can either write an article for them and post the content. Most probably, they will ask you to do a nofollow link to your website, but you are gaining a link. Though nofollow is not as powerful, it is a relevant link from a relevant website that talks about Botox and links directly to your Botox page. You see the relevancy score an educational site that talks about medspa aesthetic services, and you are writing content for them about Botox, and you are backlinking to your Botox page, which is also relevant. Everything is pure and natural. Google loves this kind of approach rather than posting 100 links from different domain authority sites throughout the world. All of those are spam links and nothing helps.

One relevant link beats 100 or even 1,000 backlinks from comments and spam.

Do not buy backlinks from: Private blog networks (PBNs), link farms, Fiverr link sellers, comment spam services, or directory spam services. Each one of them will hurt you to the core, and you can never recover from these backlinks for almost a year.

You can register your practice and add your link on medical directory websites. You can even contribute on websites like RealSelf and WebMD. You can add comments, update your listings, and do everything you can on those platforms.

There are platforms like Cermo and MDU where medical doctor influencers share content, and SoMeDocs where doctors on social media discuss topics with each other. These are platforms where doctors can register themselves and actually start sharing their content.

Doximity is also one of the top platforms where you can register and start posting your content. It is basically like a LinkedIn for doctors. You can post because when you write really good, helpful articles, you don't need to worry about whether you are posting a wrong thing or not. Even though those platforms are filled with doctors and others, you are still building relevancy. Link building from a doctor's social media, a doctor's directory, or a healthcare directory where you can list your platform all of it is totally relevant.

Guest Posting on Healthcare Publications

There are lots of websites that accept guest bloggers. You can go to Google and search "guest blogging" along with terms like "spa" or "pain diseases" or "pain doctor." You will find a lot of websites and blogs still accepting guest posts.

You can go ahead, submit your contact information, and ask them if they accept guest blogging. Once you get a reply, or you can email them directly most probably the emails will be info@thatdomain.com or contact@thatdomain.com. Just pitch 50 sites, you will land in five, and almost everyone will accept your guest post.

You can hire someone from Upwork. You just need to pay $50 to $100 per article, and most probably you will need two or three articles a month. That is going to result in 10 backlinks within five to six months like 50 backlinks within a year of trying. You are not spending a lot of money. Maximum you will be spending around $500 a month, which is going to produce 50+ high-quality backlinks that will help your actual website grow.

Instead of hiring someone for thousands of dollars locally and they will get you a thousand plus spam links that will eventually get you penalized by Google.

Local Business and Health Organization Links

This includes Chamber of Commerce, local hospital partnerships, and health fair sponsorships. You can even submit $50, $100, or even $500 for a sponsored program, and they will put your website as a top sponsor. That is basically above-$100-level backlinks to you. These relevant backlinks are something Google loves because links from your own city help a lot more than 1,000+ scammy comment links.

For example, if you are a pain doctor in Dallas and you want backlinks from your local Chamber of Commerce, you can register on sites like Dallas Regional Chamber, Greater East Dallas Chamber of Commerce, and North Dallas Chamber of Commerce. You can find more platforms through the Dallas Chamber Finder site, which will help you locate the Chamber of Commerce in your area. Register, find people, and connect.

You can also register on LinkedIn's Dallas Regional Chamber page and share your links, which will also work great for your business.

Donation-Based Backlinks: Donation websites in Dallas for example, Donation Box, public radio donation programs, Goodwill donation centers, and Salvation Army thrift store donation centers these are some of the top donation centers where you can go and donate, and eventually get backlinks from Dallas-based donation centers. You don't need to pay thousands of dollars. Even a few hundred dollars a month, or spread over the course of a year, you will be able to get good backlinks from these websites.

HARO, HERO, and Journalist Outreach

This step I don't necessarily recommend for everyone because it is a very extensive process which will cause a lot of difficulty. However, if you hire the right person to do this, you will land a great number of backlinks from news websites and journalists.

.edu Domains and Research-Based Links

You can Google ".edu domain" and "research-based link" and have the doctor submit their research papers with a backlink to the website. This will create easy, high domain authority backlinks more than 50 domain authority to your website.

For example, you can create case studies like: "We analyzed 50+ pain management patients and here's what we found." You don't need to publish very high-level research articles. You can use GPT and a bit of patient records but hide all patient details because it is against HIPAA rules and create an article like: almost all the patients are nutritionally deficient, most pain onset occurs only after age 40, patients are not taking enough supplements, and almost 50% of them are not renewing their insurance. "These are the top issues we are facing after studying 50+ patients."

This kind of content actually helps a lot with gaining backlinks from research-based websites. You just have to submit your research to those websites. Even if you pitch 5 to 10 websites a month, you will start getting links from those websites instantly.

Podcasts and Interview Links

You can Google "podcast doctors" and you will find hundreds of YouTube channels and websites that host podcast interviews. You can go to their "Be My Guest" page and enter your doctor's name, doctor's email and information or your own email as a coordinator and get the doctor on the podcast.

Most podcasts will be 15 to 30 minutes, maximum one hour. But that one hour is going to gain you one link from a good domain authority and relevant source.

What We Learned from a Recent Backlink Audit of a Pain Doctor

This pain doctor reached out to us because they were receiving 100 calls a month last year. Almost 80% of them were new patients. They were also receiving almost 2,000 visitors a month from different websites. Now they are receiving only 30 calls a month, half of them are returning patients, and only half are actually new patients.

They asked us to audit the website. We used Ahrefs, SEMrush, and got access to their Google Search Console. We audited the whole backlink schema and found these patterns:

The SEO agency that was working before was actually buying backlinks using Fiverr to increase the domain authority and bring results. What actually happened is they had been buying links from forums backlink forums where Pakistani hackers spread backlinks on Pakistani directories. These backlink providers own 100+ domains and instantly post on their own domains. Russian spam sites use software to spread spam articles across WordPress sites through link injection in the comments.

They had accumulated more than 600 backlinks. The domain authority was basically 70. I was shocked at how they could manipulate domain authority using this kind of scam links.

What We Did to Fix It

The most insane amount of work was next: we had to disavow these links. We were basically telling Google, "Ignore this link, we are not responsible." We were submitting the disavow requests again and again. It took almost 3 months to recover the traffic they were receiving earlier. We recovered almost 70% of the traffic by submitting these backlinks to Google Search Console for removal and asking Google not to consider them. We still have a lot of work to do to get them back to where they were.

The Real Cost of Bad Link Building

This is basically one year of losing traffic, one year of losing 100+ appointments a month. For a pain doctor who easily charges $500 per visit, with a lifetime value in the thousands of dollars imagine 100+ patients a month for almost a year. That is around 1,000 patients a year. On a conservative range, let's say 700 patients a year. If the lifetime value on the low end is $500, that is almost $350,000 in loss. On a conservative estimate, if even half of them are doing any procedure or something, that is going to be at least $100,000 to $200,000 of revenue for the doctor almost gone.

Just because they were using these link building platforms and people.

The Expanded Story

Each month when this pain doctor was receiving 100+ calls a month, suddenly calls dropped to 50, and the next month dropped to 30. Of those 30, 20 were pretty much recurring patients on follow-up. The doctor was shocked to see what was going on.

To handle hundreds of patients, he had hired two more registered nurses to handle the volume and follow-ups, reducing patient wait time. He even hired a residential pain doctor to help him with the whole practice. He was even thinking about taking a loan to open another satellite office in a nearby city.

But all of his dreams were shattered into pieces just because his SEO agency hired Pakistani-based link building providers to build thousands of backlinks that destroyed his entire business.

The rule is very simple. If you want to be shown in front of your patients: write relevant content for what they need to hear and read, go to the websites that produce the same type of content, and be present there by guest blogging, podcasting, or asking them to link back to you. Audit Your Entire Backlink Architecture

First, we audit your entire backlink architecture, how you have built links so far. If you don't have any links, that is great. We have a clean slate protocol that we deploy from day one.

That's it. Very straightforward, 2-3 lines. Bloggers are so busy, they don't have time to read 100+ blog post requests on a daily basis. So keep it short and simple. They know you are going to pitch them for a guest post. They are not accepting appreciation emails for the Botox content that they wrote.
